Here is a link to a generic jewel case label template.

It was created in word and you can modify it with graphics if you want.

I created my jewel cases using generic black on white. Much easier to read without all the color in the background.

I label all of mine with a Lightscribe drive. The CDRs are about 2x the price and take 25 minutes longer to burn the label, but they look amazing. I have a feeling Lightscribe will drop off the market soon though.

I have a custom made graphic with a large Stealie and the date/venue info/source info in attractive fonts that I use with the Lightscribe software. If you're interested I can send it to you. Sorry, at work, don't have any pictures.

This is probably what you want:
http://www.audio-dvd-creator.com/index.htm

That's the trial download
Trial version limit: Only the first 5 titles, and the first 5 songs in each title will be created. After you purchase Audio DVD Creator, this limitation will be removed automatically.
It's 40 bucks for the full version.
