Looks to be an Alvarez Yairi 12 string.

Cool video! Thanks for posting that link. I had to find my stub to be sure, but turns out I was at this show! It was about a year after I moved to S.F., and a good friend and guitarist from home happened to be visiting then; he wasn't a fan but thoroughly enjoyed the show and bought the CD the next day.
From the headstock logo, it's an Alvarez-Yairi, same as his cutaway six-string signature model.

The upcoming Sotheby's auction has a Guild Starfire 12-string that Jerry played. Parish says Bobby also played it.
